Draft survey description, pending verification of the sample, dates, and method.
Based on an Amari Health survey of [n] patients who began care between [start date] and [end date] and answered a follow-up survey about 90 days later ([n] of [eligible], a [rate] response rate). Percentages show the share of respondents who rated each area higher at 90 days than at their first visit. Results are self-reported, were not clinically verified, and reflect many different care plans. Individual results vary. This is not a prediction or promise of your results.
About this data. These figures come from an internal Amari Health survey, not a clinical trial. Patients who began care between [start date] and [end date] were asked to rate their energy, sleep, focus, and mood on a 0 to 10 scale at their first visit and again about 90 days later. [n] of [eligible] patients completed both surveys. Patients who stopped care or did not respond are not included, and their experience may differ. A category counts as improved when a patient's 90-day rating was at least one point higher than their starting rating. Responses are self-reported and were not independently verified. Patients received a range of care plans, so these results do not show the effect of any single treatment or medication.
Your results depend on your health history, your diagnosis, your treatment plan, and how closely you follow it. Some patients see little or no change. Treatment is available only after an evaluation by a licensed provider, who decides whether a treatment is appropriate for you. Not everyone qualifies for every treatment.
This information is for general education and is not medical advice. Talk with your provider about the benefits and risks of any treatment.
Some medications Amari Health may prescribe are prepared by a licensed compounding pharmacy. Compounded medications are not FDA approved, and the FDA does not review them for safety, effectiveness, or quality.
